It is a fair question, and the honest answer is more interesting than either side of the internet argument. Fasting does not damage healthy kidneys. What it does is change the timing of your fluid intake, the acidity of your urine, and — over months — your body composition. Each of those has a real urological consequence: one raises a specific kind of stone risk, one moves your testosterone in opposite directions depending on your starting weight, and one can make your next PSA result look worse than the last while your prostate is entirely unchanged. Intermittent Fasting and Kidney Health: The Problem Is Fluid Timing, Not Fasting
Your kidneys do not track calories. They track water and solute load. A 16:8 protocol — sixteen hours without food, an eight-hour eating window — permits water throughout in almost every version. The problem is behavioral, not biochemical: men stop eating, and then they stop drinking too, because the two habits are wired together. Coffee at 7am, nothing until noon.
When fluid intake drops, antidiuretic hormone rises and the kidney pulls water back out of the filtrate to defend blood volume. That is normal physiology working correctly. The cost is that everything dissolved in your urine — calcium, oxalate, uric acid, phosphate — gets packed into a smaller volume. Think of it as the same amount of sugar in half the glass of tea: nothing changed except the water, and now it will not stay dissolved.
Urine volume is the single strongest modifiable determinant of stone risk, which is why the American Urological Association recommends every stone former drink enough to produce at least 2.5 liters of urine daily (about 85 fl oz) [1]. That target is an output figure, not an input figure — for most men it means drinking closer to 3 liters (around 100 fl oz).
Nothing about a fasting window makes that target impossible. It just makes it easier to miss, and much harder to notice you have missed it.
Does Intermittent Fasting Cause Kidney Stones?
The best natural experiment we have is Ramadan, because it is a stricter test than any 16:8 protocol — no fluids at all during daylight hours, sustained for a month, often in high heat.
A 2024 retrospective case-control study reviewed 2,781 renal colic admissions across an eleven-year period and compared fasting Muslim patients against non-fasting controls. It found no significant increase in admissions or emergency stent insertions during Ramadan itself — but a significant rise in the month after Ramadan among those who had fasted [2]. That delayed pattern fits the clinic. Stones do not form and present in the same week. They form during the dry month and announce themselves four to six weeks later.
Heat compounds it. A Saudi study of ten years of emergency admissions found that men fasting during a summer Ramadan were roughly twice as likely to present with a stone lodged in the ureter rather than sitting quietly in the kidney [3].
The urine chemistry explains the mechanism, and it is more specific than “dehydration is bad.” Researchers collected 24-hour urine samples from 57 men before and during Ramadan fasting. Urine volume fell, as expected. But uric acid supersaturation rose significantly, while calcium oxalate supersaturation did not, and calcium phosphate supersaturation actually fell [4].
That is the finding that matters. Fasting does not raise your stone risk across the board — it redirects it. Ketone production during a fast acidifies the urine, and uric acid stops dissolving once urine pH drops below roughly 5.5. Calcium oxalate does not behave that way. So the man who develops a stone on a fasting protocol is disproportionately likely to be forming a uric acid stone — the one type that can often be dissolved medically rather than surgically.

A 41-year-old accountant came in last summer with left-sided colic and a 4 mm (about 1/6 inch) stone at the vesicoureteric junction. He had lost 11 kg (about 24 lb) on an 18:6 protocol and insisted he was drinking plenty. When we actually counted it, he was taking in around 1.5 liters (about 50 fl oz) a day, all of it compressed between 6pm and midnight. His 24-hour urine came back at 1.1 liters with a pH of 5.3 — a textbook uric acid profile. He had not been careless about water. He had been careless about when.

What Fasting Does to Creatinine, eGFR, and Who Should Not Fast
A creatinine that drifts up during a fasting period is usually pre-renal — the kidney is filtering less because there is less circulating volume, not because tissue has been injured. Rehydrate and it comes back. A man whose creatinine moves from 88 to 105 µmol/L (1.00 to 1.19 mg/dL) after six weeks of fasting has a fluid problem, not a kidney disease.
The larger evidence is reassuring but genuinely uncertain. A 2024 systematic review and meta-analysis pooled 32 observational studies covering 2,592 people with chronic kidney disease or a kidney transplant who fasted through Ramadan. It found no consistent effect on serum creatinine or eGFR in any subgroup — while stating plainly that the underlying studies carry significant limitations [5].
I read that as permission to be cautious rather than prohibitive. If your kidney function is normal, fasting is not a threat to it. If it is not normal, “the data are inconsistent” is not the same as “it is safe for you.”
Get medical clearance before starting any fasting protocol if you have an eGFR below 45, a single functioning kidney, a kidney transplant, a previous episode of acute kidney injury, or a stone currently sitting in the ureter. The same applies if you take an SGLT2 inhibitor, a loop or thiazide diuretic, lithium, or a combination of an ACE inhibitor with an ARB — every one of those either drives fluid loss or narrows the kidney’s margin for handling it.
Stop Fasting and Get Seen the Same Day If
These are not “monitor and see” symptoms. Break the fast, drink, and arrange assessment:
- Flank or loin pain that comes in waves and makes it impossible to sit still
- Visible blood in the urine, or urine the color of tea or cola
- Fever with any back or flank pain — an infected obstructed kidney is a surgical emergency
- Passing markedly less urine than usual for more than 12 hours, or none at all
- Light-headedness on standing, with a resting pulse noticeably faster than your normal
- Persistent vomiting that stops you from replacing fluids by mouth
Fasting, Testosterone, and Why Your PSA Might Rise After Weight Loss
Fasting pulls testosterone in two directions at once, and which direction you get depends almost entirely on where you started.
A 2022 review of human trials found that intermittent fasting reduced testosterone in lean, physically active young men — without any measurable loss of muscle mass or strength [6]. That is the group that tends to be most upset by the finding and least affected by it in practice. The lean 28-year-old who adds a fasting window on top of hard training is the man most likely to see a number he does not like.
The heavier man gets the opposite result, because adipose tissue converts testosterone to estradiol and losing it reverses the process. That is the link between body fat, testosterone and erectile function that explains why the same eating protocol moves two men in opposite directions. If you are fasting to lose weight and your testosterone is low because of that weight, fasting is treating the cause.
The PSA effect catches almost everybody out. A JAMA analysis of over 13,000 men undergoing radical prostatectomy showed that a higher body mass index is associated with a larger circulating plasma volume and correspondingly lower PSA concentration, while the total quantity of PSA protein in circulation stayed essentially unchanged [7]. In other words, obesity dilutes PSA.
Run that backwards. Lose 15 kg (about 33 lb) and the same amount of PSA is now measured in less plasma, so the number reads higher. Nothing has happened to the prostate. I have had men arrive genuinely frightened by a PSA that rose after a successful year of weight loss, and the correct response is not a biopsy referral — it is to interpret the result against the pre-weight-loss baseline and repeat it in six to eight weeks once weight has stabilized.
One practical nuisance worth naming: men who compress all their fluid into an evening eating window frequently develop nocturia. If you have an enlarged prostate, you have just built a protocol that guarantees you wake up twice a night.
How I Tell Men to Fast Without Wrecking Their Kidneys
I do not tell patients to stop fasting. I tell them to stop treating water as food.
- Drink 500 mL (about 17 fl oz) on waking, inside the fasting window. Plain water contains no calories and breaks no version of a fasting protocol that permits water. This single change fixes most of the problem.
- Target 3 liters (about 100 fl oz) of total fluid across the day, with at least a third of it taken before your eating window opens.
- Check your urine color at the end of the fasting window, not after dinner. Pale straw is the target. Deep amber at noon means the afternoon is already lost.
- Suspend the fast in heat. If you are training or working outdoors above 30°C (86°F), the fasting window and the heat load stack, and the summer Ramadan data show exactly where that ends.
- If you have ever passed a uric acid stone, ask for a 24-hour urine collection including pH before you start, and ask your urologist whether potassium citrate is appropriate. A urine pH below 5.5 on a fasting protocol is a stone waiting to happen.
- Book bloods at eight weeks: creatinine and eGFR, serum uric acid, and total testosterone if you have fatigue or low libido. Not at four weeks — the transient dip has not settled — and not at six months, by which point you have lost the chance to correct course.
- Do not take ibuprofen or other anti-inflammatory painkillers while fasted. Reduced renal perfusion plus a prostaglandin blocker is the combination that turns a manageable situation into acute kidney injury.
