The commonest thing I undo is unnecessary repeat scanning. A man comes in having had the same small, clearly lipid-rich adenoma scanned three times in two years, because each new report says “stable adrenal nodule, recommend interval follow-up” and no one has stepped back to note it was benign from the very first plain CT.
